Referring Information
 Hearing loss can have an enormous impact on the overall health and well being of an individual. If you believe your patient could benefit from a complete hearing and balance examination, we welcome your referral.
In most cases, insurance plans will cover the cost of a hearing and balance evaluation. The patient will need to provide us with their insurance card and a written referral from their physician.
A patient just entering Medicare Part B (medical insurance) at the age of 65 is entitled to a full physical examination, called a “Welcome to Medicare Exam”, or Initial Preventive Physical Exam (IPPE). You can make a hearing and dizziness assessment part of your initial physical exam by downloading the following documents.
